Application Process for Spring 2010


The application process for the ADN to BSN program is a two part process. You must complete your nursing department application and you must complete the University application process.

 

Please be sure to download the above information and application packet for the distance site you would like to attend. The packets above contain the nursing department application and checklist for items you need to submit to complete your application.

 

The online University application process is closed for the Spring 2010 semester so you must complete a paper university application. You can download the CSU Mentor paper application and application instructions at http://www.csumentor.edu/Support/tech_support.asp#application.

 

Spring 2010 University Application Checklist:

 

v  Print and complete pages 1-8 of the CSU Mentor Paper application which you can download from the website above.

 

v  $55 application fee: Check or money order made payable to CSUF

 

v  Official transcripts from all colleges/universities attended (if you have previously submitted official transcripts to the university or nursing department, we do have them on file and you do not need to resubmit them)

 

Please submit all the above items in one packet to:

If you have submitted a university application in previous semesters, you do still need to reapply by completing the university application process as explained above. Admissions and Records will not be able to forward previous applications for the Spring 2010 term.
