Students must submit application materials for both the University and the Department
of Nursing. Failure to do so will prevent the application from being processed.

Application procedure for admission to the Department of Nursing:
- Complete a Department of Nursing application
- Complete a Department of Nursing recommendation forms (2)
- Submit one unofficial copy of transcripts from each institution of higher education
attended
- Submit a copy of California RN license
Application procedure for admission to the University:
- Complete the University application and check or money order for $55 made to California
State University
- Submit two copies of official transcripts from each institution attended
For Laboratory and Clinical Courses, submit:
- Copy of malpractice insurance (we cannot accept policies that you may have through
your employer, they do not cover you as a student)
- Copy of CPR card
- Copies of medical clearances: PPD or chest x-ray if PPD is positive; titers for
MMR, Hepatitis B, and Varicella (state law requires titers, not vaccines)
- Copy of California driver license
- Copy of automobile insurance
