Application Checklist
|
- Students must submit application materials for both the University and the Department
of Distance Nursing. Failure to do so will prevent the application from being processed.
- If you apply after deadline noted on the cover sheet we cannot guarantee that we
will be able to accommodate all students, thus you may not be able to register for
the courses you need.
- We will begin processing and sending out materials to students after the deadline.
Late applicants may not receive all materials in time.
- It is also important that you register for courses on time and do not wait until
the first week of class. Late registration interferes with your access to course
materials, blackboard access, etc. Please work with us and register on time.

Department of Nursing application checklist |
- Department of Nursing application (included in this packet)
- One unofficial transcript from each institution of higher education attended
- Department of Nursing recommendation forms (2) (forms included in this packet)
- Copy of California RN license
- Copy of professional liability insurance (we cannot accept policies that you may
have through your employer; they do not cover you as a student)
- Copy of CPR card
- Copies of medical clearances: PPD or chest x-ray if PPD is positive; titers for
Measles, Mumps, Rubella, Hepatitis B, and Varicella (state law requires immunity
or titers, not vaccines)
- Copy of California driver’s license
- Copy of automobile insurance
