Facilities are listed below in alphabetical order. If you do not see your clinical or preceptor site listed, there are no additional requirements at this time.

The check-off sheets listed below for each specific facility are to be completed by all students. Please note that all students need to submit these facility specific documents, in addition to all School of Nursing (SON) core required documents, which are uploaded to your Castle Branch Document Manager.  Submit all facility requirements via PDF email to nursingdocs@fullerton.edu.

    IMPORTANT:  The Clinical Placement Team will email students the exact Start and End Dates of your specific clinical rotation, to allow you to complete your VALB documents.  This information should be emailed to you during the first week of July 2017, prior to your document deadline of July 13, 2017
  • VALB Check-Off Sheet (Part 2).  This will become available by June 30, 2017 and this portion is not due until the week before you start your clinical rotation  There are some documents that need to be submitted directly to your Clinical Instructor. See the Check-Off Sheet Part 2 for your instructor's contact information and to access the required documents.
    Please do not include these items from Part 2 with your packet to the Document Team (mentioned above).
