Required Documents
All nursing students need to submit the following core required documents after admission:
Please click your program PDF shown below for a specific CORE Requirements Checklist that you can print and check off as you complete:
Release Forms
Please print, review and sign (in ink) the three forms below in order to submit to your CastleBranch account.
Voluntary COVID-19 Release of Liability Waiver
In order to physically attend clinicals at University-approved sites, we must have a copy of your COVID-19 Release of Liability Waiver for the 2022-2023 academic year signed and dated, on file with your CastleBranch account. Students have the right to decline clinical placement while understanding that alternate sites may not be available during any given semester/term. For Nursing clinical courses, the Board of Registered Nursing requires a set number of in-person clinical hours. There are no alternative assignments to achieve these required in-person clinical hours. The alternative is to defer (not enroll in or drop) the Nursing clinical course until such time you feel that conditions surrounding the COVID -19 pandemic are safe for your in-person participation as well as available clinical sites.
Student handbook acknowledgment Form
All students enrolled in one of the following programs: Entry Level (Pre-Licensure), RN-BSN, or MSN/SNSC, are required to review the SON Student Handbook for their specific program at the beginning of every academic year. You can locate your program's handbook at the Current Student Page. The Student Handbook Acknowledgment form is available within the handbook for your specific program.
Students will need to review, sign, date, and upload the Student Handbook Acknowledgment form to their Castle Branch account annually on or before August 15. Please ensure that your full name is written on your form or it will not be approved. The form must indicate the current academic year (i.e. “2022-2023”).
Background Check Statement
Ordering a Castle Branch account includes a background check. Per the Student Handbook, the CSUF School of Nursing follows the Orange County/Long Beach (OC/LB) Consortium for Nursing position statement on background checks unless a partnering agency or entity maintains stricter and/or differing requirements and/or are not OC/LB Consortium members.
Drug Screen
All Pre-Licensure students and MSN Women’s Health Care students are required to complete drug screening as indicated below. Other Post-Licensure students may be required to complete drug screening as required by clinical facilities. The drug screen will test for the following: amphetamines, barbiturates, benzodiazepines, cocaine, cannabinoids, meperidine (demerol), methadone, methaqualone, opiates, oxycodone, phencyclidine (angel dust), propoxyphene.
A 10+ panel drug test is required prior to registration to clinical nursing courses. Castle Branch currently contracts with Lab Corp laboratories to conduct drug testing and the student will be assigned to a site closest to residence for testing. Both are federally approved labs. Drug test results will be automatically posted to the student’s Castle Branch account. You will not need to upload a separate form. The initial drug screen satisfies this requirement during continuous enrollment in the program; should the educational process be interrupted or if required by clinical facility, a new drug screen will be required.
Mandatory Reporter/Training Verification Form
View the three HIPAA training videos, review the seven PDF modules and complete the corresponding quizzes at the beginning of every academic year.” Once you have viewed all the videos and completed all the quizzes with 80% passing rate, the Mandatory Reporter/Training Verification Form will be available for you to fill out and download. Please print, sign, and upload the form to your Castle Branch account. If you have any technical issues please contact nursingweb@fullerton.edu.
