Medical Records
CPR (BLS card)
Your CPR certification must include the Basic Life Support (BLS) for Healthcare Providers Course and be completed through the American Heart Association (AHA). You will need to submit a copy of your electronic BLS card, it must include your name and expiration date for approval.
Please visit the American Heart Association HeartCode BLS website to find classes near you. Classes must include the in-person skills portion as well as the online portion.
Flu Vaccination
You are required to submit a copy of your flu vaccination annually to Castle Branch. The renewal date is set every October 15th for all students. If you do not prefer the standard flu shot, the intranasal flu-mist is also acceptable.
Documentation MUST clearly display student’s name, date of vaccination, manufacturer, and lot number.
No declination/waivers will be accepted. We only accept medical contraindications as a reason for not taking the flu vaccine. The medical contraindication (anaphylaxis, Guillian-Barre, or other serious reaction) must be documented by your physician or healthcare provider.
Health Insurance
Submit a copy of your current health insurance card or proof of coverage to your Castle Branch account. If your name is not shown on your card, verification of coverage from your insurance provider is required.
For more information or to apply for health insurance, please refer to the Covered California website. Medi-Cal is one option for students. It is a free or low-cost health coverage plan for children and adults with limited income and resources. The enrollment period for Medi-Cal is open all year long.
Physical Exam Form/ Health Clearance
The Health/Physical Exam Form is required for all Nursing students to be completed by a healthcare provider. The Health/Physical Exam Form must be completely filled out, signed, dated, and stamped by your doctor or healthcare provider (and must include his or her printed name and medical license number) to be valid and approved in Castle Branch. Ensure to review and show your healthcare provider the last two pages for detailed information.
Tdap Booster
Submit documentation of a current Tdap booster (within the past 10 years) to your Castle Branch account. The renewal date will be set for 10 years from the administered date of your booster.
TB Clearance
All students are required to submit a copy of either a negative PPD (TB skin test), QuantiFERON Gold Blood Test, T-Spot TB Test, or a healthy chest X-Ray interpretation to Castle Branch. PPDs, QuantiFERON Gold Blood tests, and T-Spot TB tests are valid for one year. Chest X-Rays are valid for two years.
The two forms listed below are based upon the individual needs of a student's specific program or health status:
TB 2-Step Form (for Pre-Licensure Students only)
Complete the following TB 2-Step Form during your first semester. This means you must complete TWO separate PPD skin tests, 1-3 weeks apart. Ensure that your name is printed at the top of the page before submitting your completed form to Castle Branch. The 2-Step form is only required once. A renewal date will be set for one year from the "read by" date of your second PPD skin test on this form. From then on, a renewal will be set to complete the standard PPD skin test once a year.
For students who have tested positive for TB: Rather than completing the TB 2-Step process above, you will be required to submit a copy of a recent chest X-Ray letter or lab report stating that you have clear/healthy lungs and do not show signs of active tuberculosis.
TB Questionnaire (only for students with a history of positive PPD)
Do you have a history of a positive PPD? If yes, submit the following TB Questionnaire. The renewal date will be set for 1 year from the date on your questionnaire. If you have only had negative PPD results in the past, please answer "no". No renewal date will be set.
Ensure that your name is clearly written or typed at the top of the form. Forms without a name will be rejected.
Titers
All students are required to submit positive titers for the 5 items below. Your titer records must indicate your level of immunity to Hep B, Measles, Mumps, Rubella and Varicella. If you submit any negative titers (indicating that you are not immune) you will be required to complete follow-up vaccines/booster shots. Details are listed under each requirement below.
Attention: If you are currently in the middle of a vaccination series (i.e. Hepatitis B) and cannot go in for your titers yet, please upload your current vaccination records. Custom renewal due dates will be set specifically for you, depending on how far along you are in the series.
Hepatitis B
Submit a copy of your positive, immune, or reactive Hepatitis B titer. If your titer is negative, equivocal or non-reactive ensure that documentation of your prior vaccination or history of Hepatitis B disease is included with your titer record. New alerts will then be created for you to acquire another round of the 3-dose immunization series and upload each document as they become due (second shot is due 1 month later, then third shot is due 5 months later). A final titer will be required 1-2 months after your third vaccine.
Measles
Submit a copy of your positive, immune, or reactive Measles (Rubeola) titer. If your titer is negative, equivocal or non-reactive, ensure that documentation of your prior vaccination or history of Measles (Rubeola) disease is included with your titer record. New alerts will then be created for you to acquire the 2-dose immunization (booster) series and upload each document (one month apart) for approval.
Mumps
Submit a copy of your positive, immune, or reactive Mumps titer. If your titer is negative, equivocal or non-reactive ensure that documentation of your prior vaccination or history of Mumps disease is included with your titer record. New alerts will then be created for you to acquire the 2-dose immunization (booster) series and upload each document (one month apart) for approval.
Rubella
Submit a copy of your positive, immune, or reactive Rubella titer. If your titer is negative, equivocal or non-reactive ensure that documentation of your prior vaccination or history of Rubella disease is included with your titer record. New alerts will then be created for you to acquire the 2-dose immunization (booster) series and upload each document (one month apart) for approval.
Varicella
Submit a copy of your positive, immune, or reactive Varicella titer. If your titer is negative, equivocal or non-reactive ensure that documentation of your prior vaccination or history of Varicella (Chicken Pox) disease is included with your titer. New alerts will then be created for you to acquire the 2-dose immunization (booster) series and upload each document (one month apart) for approval.
COVID-19
COVID-19 Vaccine and Booster Requirement:
Please submit documentation of your COVID-19 Vaccine/s and Booster to your CastleBranch Account. Documentation must include the vaccine manufacturer.
COVID-19 Booster:
The CDC provides a table which breaks down the timeframes and recommended booster based on your initial COVID-19 vaccine/s.
Medical or Religious Exemption
Please refer to the CSUF Titans Return: COVID-19 Recovery for the most current information and guidelines. COVID testing required. Please refer to the specific guidelines for face-to-face, online, and clinical facilities.
Misplaced or Lost COVID-19 Vaccine Card
- Contact your vaccine site and they may be able to re-issue you a new vaccine card on the spot or

Additional Fees
Additional fees for clinical/practicum courses may apply. Students may be required to order an additional Castle Branch tracker(s), CB Bridges account, COVID-19 tests, etc. depending on their clinical/practicum site. If fees are required, students will be notified by the Clinical Placement Team and given appropriate instructions.
